I have created this behavior chart for use with Special Education students and students who are on RTI for behavior issues. I have used this chart for both purposes within my primary classroom (K-2).
Special Education: This chart provides the student with a schedule of their day. It breaks down the time frame for each activity to help the student monitor his/her behavior and task completion. It also serves as an excellent form of documentation.
Response to Intervention: This chart provides the student with a segmented time frame for each activity. The chart allows the student to begin monitoring his/her behavior and encourages self-assessment for each activity. The teacher is able to view what areas and time of day the student is showing behavior problems. It is an excellent form of documentation for RTI. For data collection, add the score for each day and divide by the total number of points possible to get the percentage of time on task.
This packet includes an example of a chart with activities and time frames. It also includes a generic chart of activities only.
