Have you been wanting to have a way to efficiently record the observations you make as you conference and meet with your readers? 

We began a new curriculum this past year and after many discussions during Professional Learning Community meetings I came up with a series of options in the form of a mini-book. There are many options because there were many different ideas about how this could be useful -- so there is not one but five variations -- you can decide which version works best for you.

These came in so handy over the course of the year because as I used them I could easily refer back to see what books the children were reading, where they were successful and where they needed more coaching. The reading specialist worked out of the same book so we really were able to work effectively and collaborative in targeting specific needs of our struggling learners.

These are designed to be printed landscape and back to back. Directions for constructing them are included in the download.

This naturally aligns with the Common Core and is applicable to any grade where you are conferencing and conducting small group instruction.
